Hand addressed mailed copy with mailing marks and mailing wear, folded in two as issued with holes from a removed staple in upper left corner. 2 cm. tear to upper left corner of page and 9 mm. tear to top edge; 6 mm. tear to right side edge; 1.3 cm. tear to bottom edge; and 2 mm. tear to left side edge.
